Overview:
 
An Application for compensation must be filed with a cover sheet Local Form 2016-1 and a Proposed Order.  When required by Local Bankruptcy Rule 9013-3, a Certificate of Service must also be filed.  The person requesting compensation must have been appointed by court order.
